Student Registration

Required Forms

All required forms must be printed, filled out and returned to the school to complete registration.
Registration will be Tuesday, August 8 and Wednesday, August 9, 2017 at Morgan Middle School. Please plan to register between 7:30-11:30 or 12:30-3:00.

New Student Registration FormRegistration Information on new students to Morgan School District.

Notification Information Form – Morgan Middle School will use our e-mail notification & automated phone systems to inform parents and students of important information throughout the school year. This form must be completed to ensure notification from the school.

Race & Ethnicity Form – The US Department of Education requires all schools to collect information on student race & ethnicity each year. Even if this information has not changed, this form must be completed.

17-18 Acceptable Use Policy– Students will have the opportunity to use computers and, at times, access the internet. In order to be given this privilege, this form must be signed each year and put on file at the school.

Media Release Form -This form gives permission to have the student’s name and photo appear in the media for academic and/or athletic purposes and also gives permission to be interviewed by the press.

17-18 Medication administration authorization form – Required only if student has a medical condition that the school needs to know about.

USIISRequired by the state for each student to access immunization records only.

Final Schedule and Locker Information will be given to students/parents after registration documents have been turned in at MMS Office on Registration Day (Tuesday, August 8 and Wednesday, August 9, 2017)

School Pictures – Picture day will be Monday, August 28, 2017 at the Middle School. All students are required to get their pictures taken for their student activity cards. If your child is absent on August 28th, picture retake day will be Tuesday, September 26, 2017.
